Risk Factors have the potential to negatively impact a patient's overall health and well-being. Risk Factors are developed based on the patient's determinants and reported information, which is a component of the Well-Being Assessment. They are updated regularly via the well-being status. Risk Factors are typically addressed by assigning Care Goals and interventions and activities in the Care Plan.

Modifiable Risk Factors

  • Physiological Measures - High Cholestrol, Diabetes, Hypertension, Obesity, etc.
  • Behavioral Risk Factors - Smoking, Physician Activity, Nutrition, etc.
  • Care Gap Risk Factors - overdue preventitive, screening and testing interventions

Non-Modifiable Risk Factors
